According to the commission circular No. 09/2021 of the University Grants Commission, you will not be given any opportunity to correct your name after the effective date of your degree.
On that reason and to avoid the inconveniences in the process of writing the names of the graduands on the degree certificates, all the students of the final year of FAS should send the scanned copies of the following documents in PDF format {(must be clear scanned documents (not images, photos taken from cameras, mobile phones or shade/dark documents scanned by mobile phones and etc. )} on or before 04.04.2024 via e-mail (con.fas@sjp.ac.lk).
- Duly filled form. Download the form here
- A scanned copy of the first two pages of the Student Record Book to certify your name in the university registration.
- Certified copy of the birth certificate in Sinhala / Certified copy of the Sinhala translation of the birth certificate (check name, whether it is exactly the same as the name written on the form)
{Subject of the e-mail should be as follows:
: Index number, Registration no, Name of the Degree
Ex : AS2019xxx, AS95xxx, Bachelor of Science Honours Degree in Zoology}

The name you have written on name form should exactly be the name appearing on your Birth Certificate / Sinhala translation of the birth certificate.
In the original degree certificate, your name in Sinhala will be appeared as on your birth certificate / Sinhala translation of the birth certificate and in English, it will be appeared as in the final results. Since the name in the final results same as the name appeared in your Student Record Book, check your Student Record Book for any name correction or any spelling correction in your name and get it corrected from the Academic Student Affairs, Publications & Examinations Division before sending the above documents.
You have been previously informed about the matter of name correction mentioned above by the Academic Student Affairs, Publications & Examinations Division. If there are any students who have not got their name corrected yet, please do so before the effective date of your degree. According to Commission Circular No. 09/2021, you are not allowed to apply for name corrections in university registrations after the effective date of your degree.
Since the name of your degree certificate will not be corrected for any reason, the name on your other essential documents such as educational certificates, Passport should be the same as the name on the Student Record Book (name of the university registration).
